This Form is the 2025-2026 REQUEST FOR RECRUITMENT - Phase 1 for Fraternity and Sorority Life (FSL).
This form initiates the process of recruitment/intake for new members to a UIC fraternity and sorority chapter. The purpose of this document is to provide the fraternities and sororities of the UIC Fraternity and Sorority Life (FSL) Community, their Advisors, and prospective members with a source of information regarding Recruitment/Intake and the Schedule for New Member Education.
Failure to submit this information prior to any interest meeting/recruitment event could put your chapter in jeopardy of having to reschedule your meeting(s)/event(s) or delaying your schedule of your intake and/or new member education program.
** BEFORE YOU BEGIN, visit the FSL Resources tab on the FSL website and download copies of both Phase 1 documents here:
https://fsl.uic.edu/resources-events/resources/
Chapters must adhere to the following guidelines if they are to conduct recruitment/intake/new member education at UIC:
